What is my role as Community Health Worker?
- Perform personal support tasks reflecting the needs of the client and family
- Assist our patients with their activities of daily living tasks which may include:
- assisting the client in walking,
- climbing or descending stairs,
- getting into and out of bed,
- eating, dressing and other matters of personal hygiene.
- Provide cognitive, social and/or emotional support to the client
- Aid in personal care
- Provides companionship and support
- Light cleaning, preparation of nutritious meals, care of clothes, shopping for the client as required, and monitoring and maintaining household safety.
- Provides simple bedside care, where required,
- Monitoring and maintaining household safety.
- Assesses the client’s pattern of communication and ability to relate
