We are seeking a Nurse Practitioner with a specialization in geriatric medicine to join our healthcare team. This role requires a strong leader and advocate for patient and family-centered, elder-friendly care. The ideal candidate will be instrumental in shaping and promoting policies and processes that prioritize the well-being of elderly patients. The Nurse Practitioner in this role will be responsible for conducting comprehensive geriatric assessments (CGAs) to thoroughly understand the medical, functional, and psychosocial needs of elderly patients at the Foothills Medical Centre. They will develop and implement tailored care plans that address these multifaceted needs, ensuring a focus on holistic patient care. An integral part of their role involves working closely with the geriatrics team and contributing to the Hospital Elder Life Program operating at the site. As a strong advocate for patient & family-centered and elder-friendly care, the Nurse Practitioner will provide expert advice and consultation to units and site leadership on developing and implementing elder-friendly policies and processes of care. They will lead initiatives aimed at enhancing the quality of life for elderly patients and will be responsible for offering recommendations for managing complex geriatric health issues. Educating patients, families, and caregivers about health conditions and care strategies is another critical aspect of their role. The Nurse Practitioner will also be tasked with maintaining thorough and accurate medical records and leading collaborative efforts as part of multidisciplinary teams dedicated to geriatric care.
Description:
As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:
Successful completion of an approved Nurse Practitioner program and a minimum of master’s degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Master’s degree in nursing. Three years of experience in a geriatric’s specialty program (e.g., specialized geriatrics, supportive living, or equivalent. Three years of experience as a Nurse Practitioner. Proven leadership skills and experience in advocating for elder-friendly care and policies. Strong ability to provide expert advice and consultation to healthcare teams and leadership. Exceptional communication skills, capable of effectively engaging with patients, families, and healthcare professionals. Deep commitment to providing compassionate, patient and family-focused geriatric care. Ability to work independently and collaboratively within a multidisciplinary team. Preferred Qualifications:
Current CNA certification in Gerontology. Demonstrated pattern of professional development and continuous improvement in Geriatric Medicine.
