Job description: Your Opportunity:

We are seeking a Nurse Practitioner (NP) for the Narcotic Transition Services program. The NP is responsible for assessing, diagnosing, treating, and providing medical oversight of the clients in the program. The NP supports Case Management of the clients in the program including Narcotic Transition Services and Opiate Dependency Treatment through local and provincial resources including the Virtual Opiate Dependency Program. The NP provides a Primary Care perspective on clients accessing the service. The NP works collaboratively with other members of the team, other Addiction and Mental Health programs, other AHS programs, and other non-AHS programs to create connections and support ongoing care of clients. The NP has an expert understanding and knowledge of addiction and mental health programming, strong collaborative skills, and the ability to liaise with other internal, allied, contracted, and community stakeholders to facilitate timely care to individuals experiencing problems related to alcohol, opioid, other drug, tobacco, and co-occurring addiction and mental health problems

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:

Successful completion of an approved Nurse Practitioner program and a minimum of master’s degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Additional Required Qualifications:

Meet requirements for education and/or preceptorship for the category of prescribing of Suboxone® and methadone applicable to their practice. Preferred Qualifications:

At least 2 years nursing experience in an Opiate Dependency Treatment program, Supervised Consumption Site, or other relevant Addiction and Mental Health setting within the last 5 years is preferred. Ability to create and manage complex care plans. Computer proficiency preferred. Experience providing support to vulnerable clients preferred.
