Practicing within the philosophy and guidelines of the Southern Alberta Renal Program, the Registered Nurse works as a member of the health care team, collaborating on the delivery of holistic, comprehensive care to meet the needs of individuals, families and/or communities. Under general supervision of the manager/supervisor, the Home/Nocturnal Nurse is responsible for implementing evidence-based best practice, primarily in, but not exclusive to, the area of home/nocturnal hemodialysis. This position is responsible for providing training and monitoring/optimizing care for renal patients who are candidates to pursue the independent self-care modality of home/nocturnal hemodialysis. This includes assessing and recruiting patients for home modality capability. It encompasses assessment of diverse renal populations, from those requiring early intervention for chronic kidney disease (e.g. modality education to recruit patients before they require dialysis) to those who are already on conventional in-centre hemodialysis. This position involves lifting and movements requiring fine motor skills (e.g. hand dexterity). This position requires occasional patient home visits. A vehicle is not required. Staff may use a personal vehicle (mileage will be reimbursed), a vehicle may be provided for use, or alternate means of transportation will be supplied where required. This position may require Days, Evenings and Weekends.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour – $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management – Chapters 1 – 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0) Additional Required Qualifications:

Ability to work independently in a rapidly changing environment required. Computer skills required. Ability to maintain effective and respectful working relationships required. Demonstrated excellent interpersonal, organizational and assessment skills, and the ability to work effectively as a team player required. Minimum 2 years of current nephrology nursing experience required. Recent or current acute and/or chronic hemodialysis experience required. Ability to successfully complete the Specialized Clinical Competency of Hemodialysis required. Previous patient teaching experience required. Ability to be on call outside of regular hours required. Charge Nurse ability and experience required. Preferred Qualifications:

Recent or current Home Hemodialysis experience strongly preferred. CNeph(C) qualification preferred. Successful completion and application of formal coursework or education in adult teaching preferred. Experience with ongoing, continuous case-management of patients with chronic disease preferred.
