Reporting to the Area Manager, Seniors Health, the Care Manager is responsible and accountable for direction, team leadership, and organization over the day to day activities of the High Prairie Long Term Care and Designated Supportive Living departments. The Care Manager will be responsible and accountable for making unit-level decisions and having direct leadership responsibility for contributing to the achievement of the goals of the client and resident care service. This position will require strong relationship building abilities that include facilitation, collaboration, and consultation skills. If you can utilize persuasion and influence in order to successfully lead these departments, this position may be for you. High Prairie, in Northern Alberta’s Big Lakes County is known as the “Gateway to the Mighty Peace Country.” With a population of 2,600 people, there is an abundance of beautiful open plains, rich farmland, and thriving agriculture, forestry, oil and gas, and service industries. Unlimited recreational activities are available in town including golf, a professional rodeo and a variety of winter sports including sled dog races every February. A quick 20 minute drive will bring you to the stunning Lesser Slave Lake area, one of Alberta’s top recreational playgrounds and host to the annual “Golden Walleye Classic” fishing tournament. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. As the Integrated Care Manager (ICM) – Designated Supportive Living and Long Term Care, ICM will be responsible for providing leadership to an interdisciplinary team of healthcare professionals dedicated to providing leading care to Albertans. ICM leadership, direction and organization of these departments will be in support of the vision, mission and business plan of AHS. As the Integrated Care Manager, you will be making key decisions and establishing and contributing to the achievements and goals of the Seniors Health portfolio. As the Manager, you will be responsible for developing and maintaining strong working relationships with key stakeholders, facilitating change management, promoting team development, teamwork and coordinating staffing and clinical work as identified by the Area Manager. You will be responsible for developing and maintaining an effective framework for the operations of patient care services in order to facilitate the delivery of efficient, effective and safe patient care. You will be implementing and monitoring your portfolio’s budget while undertaking risk management, quality assurance and utilization of information in this field. As the successful candidate, you will have experience successfully communicating with different stakeholders and demonstrate a client-centered approach. We are also looking for someone who has the ability to develop creative approaches to client and resident care and who demonstrates sound critical thinking and ethical decision making.

Required Qualifications:
1-2 years previous management experience. Bachelor’s Degree in a related health services field. Minimum of 5 years Nursing (or health services related) experience.

Preferred Qualifications:
Seniors Health experience. Experience with Meditech, RAI, CCHSS, and Accommodations Standards. Home Care experience.
