This is a multi-diverse caseload position offering services to Acute Care, Home Health and Chronic Disease Management (CDM). Restricted Activities A and C are required. Diabetes experience preferred.
Duties and responsibilities include:
- Providing nutrition care and support to individuals, patients, residents and groups.
- Providing nutrition services through: nutrition assessment; care planning and charting; intervention, goal setting and monitoring; counseling and education programs.
- Consulting and communicating with the multidisciplinary care teams.
- Acting as a resource to other health professionals and communities within Interior Health.
- Providing input to the Professional Practice Leader by participating in the development of nutrition practice guidelines and procedures; research and continuous quality improvement initiatives for the facility and program.
Schedule is Monday through Friday Days and access to the use of a personal vehicle is required.
