Job description: About the Role

As a Community Inclusion Worker, you will assist a person who requests support to participate in personal, social, vocational, and volunteer opportunities within the community. The services are determined by the person and based on their specific goals and preferences. You’ll encourage and empower the person to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work collaboratively towards a goal will allow you to influence your person’s success.

Core Responsibilities

Communication and Report Writing

  • To assist in implementing and writing up personal planning for each individual in conjunction with the coordinators/ Managers, family and the other professionals
  • To record relevant information in the individuals, staff and any other communication books
  • To report immediately to the coordinators/ Managers any lapse in community standards of care, health and safety concerns; whether the occurrences were voluntary or involuntary
  • To be responsible for relaying information, writing, and checking communication books and keeping all information up-to-date
  • To be responsible for proper financial record keeping and accounting for petty cash
  • To communicate with individual’s families as required
  • To record any encounters with people in the community who could possibly from future friendships with an individual

Service

  • To relate to the individuals in a caring positive manner; which promotes feelings of personal worth and dignity
  • To focus on the developmental potential which acknowledges the individuals rights to self- determination and privacy
  • To ensure the individuals are involved in the numerous decision making situations in the home and community
  • To support the individuals in all areas of personal growth directed towards their becoming as self- sufficient as possibility inclusion worker

You will have

  • Grade 12 education
  • Community Support Worker Diploma or equivalent
  • Minimum 2 years of experience in community social services/health care
  • Current Emergency First Aid/CPR and Food Safe certificates
  • Behavior management training
  • Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members
  • Non-verbal communication skills
  • Valid Class 5 BC Driver’s License
  • Satisfactory Driver’s Abstract & reliable vehicle with $2,000,000 third-party liability insurance
  • Criminal record clearance for children and vulnerable adults
  • Proof of COVID-19 vaccination status. As per current Public Health Orders as of October 26, 2021, all employees working for PCCR must be fully vaccinated for COVID-19.
  • Moderate computer literacy
  • Eligibility to work in Canada
