Position Summary

Toronto Western Family Health Team (TW FHT)’s program links with the surrounding community to provide primary care to families and community members, with special attention to the needs of the aging population, children and women. With the teamwork of physicians, nurse practitioners, primary care nurses, social workers, pharmacists, physiotherapists, occupational therapists and chiropodists, we focus on supporting the a broad range of health of health concerns.

Duties

  • Ensure the accuracy, timeliness, and completion of various reports and data required by functional groups within the TW FHT
  • Provides expertise and support to achieve optimal and timely monitoring, maintenance and updating of all TW FHT data management systems
  • Acts a support resource to staff within the FHT for the investigation and timely resolution of system data problems
  • Audits data contained within the materials provisioning database and other data management systems
  • Document user requirements and operating policies and procedures
  • Contributes to the Examination of Data Integrity and Output by monitoring error logs and outstanding lists and investigates items
  • Recommend or develop solutions, where appropriate, to prevent re-occurrence of problems
  • Participate in the design and generation of queries and reports to facilitate the monitoring/investigation of data by management and clinical staff
  • Manages the acquisition, installation and maintenance of workstations, communication devices, printers and other equipment/information systems required for the operation of the department

Qualifications

  • Completion of a Bachelor’s Degree program, Computer Science, Computer Engineering, Health Informatics, or recognized equivalent
  • 1 year formal experience in data management
  • 1 year experience working in a Windows environment
  • 1 year MS Office Suite experience
  • Health care environment/sector experience, an asset
  • Very good analytical, process analysis, problem recognition, problem solving, interpersonal, verbal and written communication skills
  • Ability to extrapolate, conceptualize and express process improvements
  • Ability to effectively deal with high priority unscheduled tasks and problems
  • Understanding of programming techniques and languages
  • Detailed understanding of database management, preferred
