This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.

Responsibilities

Reporting to the Primary Health Care Team Lead, the incumbent will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. As a member of a primary health care (PHC) team, the PHC Dietitian provides a range of PHC services. These services include, but may not be limited to, health promotion, illness prevention, screening, treatment and monitoring of people living with chronic health conditions. Expertise may also be required to support people living with other chronic health problems, community development and advocacy. The incumbent will be required to collaborate with others to increase community capacity as an educator, researcher and clinical practitioner. These services are provided within a holistic, client/family centred care framework.

The incumbent will provide health assessment, interventions and appropriate follow up in accordance with acts, regulations, policies, standards, guidelines, mission and objectives of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness – Province of Nova Scotia and the Nova Scotia Health Authority.

Qualifications

  • Bachelor of Science in Human Nutrition from a recognized university
  • Dietetic internship (a 1 year integrated or post graduate program)
  • Current Membership in Nova Scotia Dietetic Association required
  • Annual continuing education to maintain Nova Scotia license
  • Two years’ clinical experience preferred
  • Membership in Dietitians of Canada preferred
  • Current CPR/BLS certification is required
  • Designation as a Certified Diabetes Educator preferred
  • Experience with group education
  • Demonstrated time management and organizational skills
  • Familiarity with principles of Motivational Interviewing
  • Exhibits creative problem-solving ability; Ability to work collaboratively with other disciplines and committees to achieve excellence in patient care
  • Computer proficiency in MS Office( Word, PowerPoint), email and internet
  • Ability to work well independently as well as within a multidisciplinary team environment
  • Excellent verbal and written communication skills
  • Effective research and analytical thinking skills
  • Competencies in other languages an asset; French preferred

Hours of Work

Permanent, Full-time position; 75 hours bi-weekly

This position involves working 50% at the New Waterford Collaborative Family Practice in New Waterford and 50% at the Sterling Health Centre in Glace Bay

Salary Information

$30.8936 – $40.7231 hourly
