Position Summary:
Contact tracing is a process that is used to identify, educate and monitor individuals who have had close contact with someone who is infected with a virus. These individuals are at a higher risk of becoming infected and sharing the virus with others. The goal of contact tracing is to limit the spread of COVID19 but finding and isolating cases
Responsibilities:
- Co-ordinates and monitors strategic clinical infectious disease control activities including case management, contact tracing, surveillance, results reporting, and screening
- Call contacts of newly-diagnosed patients and staff to investigate and maintains surveillance reports of COVID19 cases
- Communicate with contacts in a professional and empathetic manner
- Maintains confidentiality of patients and staff
- Assists with outbreak management, as recommended in collaboration with Public Health, IPAC and EHSW
- Participates in the development and evaluation of education materials
- Participates in continued learning opportunities about infectious disease prevention and control
- Establishes and maintains a close working relationship with leadership and staff to build capacity, promote awareness and self-determination in the prevention and management of COVID19
- Assists in preparation of reporting on contact tracing details and areas for improvement for the protection of staff and patients
Qualifications
- Regulated health professional, registered nurse preferred
- Minimum 3 years of recent experience in a clinical role,
- DOHN and COHN (C) credentials an asset
- Ability to exhibit a professional, positive attitude and work ethic
- Excellent interpersonal skills; ability to interact professionally with culturally diverse individuals during a time of crisis and distress
- Ability to show empathy and compassion to distressed individuals
- Excellent organizational and verbal communication skills
- Demonstrated ability for critical thinking and sound judgment required
- Ability to handle confidential information with discretion and professionalism
- Manages priorities effectively
- Experience in program planning, implementation and evaluation
- Understanding of PHO and MOH guidelines related to COVID19 and communicable diseases
- Excellent computer skills with various software, including Microsoft Office and EPIC
- Experience with case management and contact tracing of COVID19 or other communicable disease an asset
- Experience with Disability management
- Ability to facilitate and lead group discussions, and implement feedback into practice
- Ability to liaise with external agencies, partners and other organizations
- Ability to develop and analyze relevant occupational health & safety reports
- As part of a dynamic team of professionals must be flexible team player with strong interpersonal and communication skills
