Position Summary:

Contact tracing is a process that is used to identify, educate and monitor individuals who have had close contact with someone who is infected with a virus. These individuals are at a higher risk of becoming infected and sharing the virus with others. The goal of contact tracing is to limit the spread of COVID19 but finding and isolating cases

Responsibilities:

• Co-ordinates and monitors strategic clinical infectious disease control activities including case management, contact tracing, surveillance, results reporting, and screening
• Call contacts of newly-diagnosed patients and staff to investigate and maintains surveillance reports of COVID19 cases
• Communicate with contacts in a professional and empathetic manner
• Maintains confidentiality of patients and staff
• Assists with outbreak management, as recommended in collaboration with Public Health, IPAC and EHSW
• Participates in the development and evaluation of education materials
• Participates in continued learning opportunities about infectious disease prevention and control
• Establishes and maintains a close working relationship with leadership and staff to build capacity, promote awareness and self-determination in the prevention and management of COVID19
• Assists in preparation of reporting on contact tracing details and areas for improvement for the protection of staff and patients

Qualifications

• Regulated health professional, registered nurse preferred
• Minimum 3 years of recent experience in a clinical role,
• DOHN and COHN (C) credentials an asset
• Ability to exhibit a professional, positive attitude and work ethic
• Excellent interpersonal skills; ability to interact professionally with culturally diverse individuals during a time of crisis and distress
• Ability to show empathy and compassion to distressed individuals
• Excellent organizational and verbal communication skills
• Demonstrated ability for critical thinking and sound judgment required
• Ability to handle confidential information with discretion and professionalism
• Manages priorities effectively
• Experience in program planning, implementation and evaluation
• Understanding of PHO and MOH guidelines related to COVID19 and communicable diseases
• Excellent computer skills with various software, including Microsoft Office and EPIC
• Experience with case management and contact tracing of COVID19 or other communicable disease an asset
• Experience with Disability management
• Ability to facilitate and lead group discussions, and implement feedback into practice
• Ability to liaise with external agencies, partners and other organizations
• Ability to develop and analyze relevant occupational health & safety reports
• As part of a dynamic team of professionals must be flexible team player with strong interpersonal and communication skills
