It is a time limited, multi-disciplinary, goal oriented and client-centered approach. Restorative care helps clients, across the continuum of care, return to home and remain at home as long as possible. It mitigates the risk or need for higher levels of health care or premature admission to a Continuing Care living option. Restorative care helps medically stable clients learn or re-learn the necessary skills required to complete their daily living activities. Restorative care can also refer to a health service setting (see above description) The services will be provided by an integrated team that includes a case manager, regulated health professionals such as OT, PT, SW, a Nurse Practitioner and rehab assistants with expertise in working with the frail elderly population and/ or adults living with complex chronic conditions.
