This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.
Responsibilities
Reporting to the Primary Health Care Manager, the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care and Department of Family Practice portfolio. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders to ensure localized support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSHA departments, external individuals and partner agencies.
This position will focus primarily on the Chronic Disease Management and Wellness Program, access, virtual care and provide general project support to other practice support and primary health care initiatives
The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support change management strategies that are inherent in the services plan for primary health care.
Qualifications
- Baccalaureate degree preferably from a health discipline/health related science
- Current registration with professional association if applicable
- Two years’ of recent related experience
- Chronic Disease Management experience preferred
- Coordination and Project Management Skills
- Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management.
- Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset.
- Experience working in a family practice setting with family physicians an asset.
- Organizational and project management skills. Superior time management skills in order to prioritize and complete assignments.
- Sound knowledge and skill working in an interdisciplinary settings.
- Ability to analyze information to identify priorities and emerging issues.
- Superior communication skills – written, oral, presentation.
- Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills.
- Understanding of Behavior Change Techniques.
- Competencies in other languages an asset, French preferred.

Hours of Work
Short Assignment, Full-time position; 75 hours bi-weekly

Salary Information
$30.3032 – $36.7885 hourly
