Reporting to the Manager, ACTT, the incumbent will work closely with the interdisciplinary team to deliver treatment, rehabilitation and support services to clients living in the community. The RN on the ACT team brings both a specialist and generalist role to the team. The scope of the role includes: practicing case manager of a small group of clients, expertise in medication support and health promotion, as well as assistance with activities of daily living, psychosocial goals, family support and facilitation of linkages with community partners.
Requirements:
- Current certification with the College of Nurses of Ontario as a Registered Nurse
- Current CPR/BCLS certificate and evidence of continued education required
- Minimum 3 years recent experience in a community mental health setting working with clients who require careful monitoring and evaluation of the effects of psychotropic medications and working with community resources
- Current clinical skills and experience to assess, plan, develop, co-ordinate and provide treatment, rehabilitation and support services to program clients with severe and persistent mental illnesses
- Thorough understanding of the ACTT model
- A clear understanding of the characteristics and problems of seniors with severe and persistent mental illnesses
- Understanding of working with seniors with addictions, cognitive impairments and physical limitations.
- Experience and belief in the value of working in and committing to a fully integrated multidisciplinary team and interdisciplinary model, including clinical guidance and peer support
- Good working knowledge of the Mental Health Act
- Demonstrated understanding of Long Acting Injectable (LAIs) antipsychotic medications
- Demonstrated excellence in interpersonal and communication skills that facilitate interaction among all members of the ACTT
- Demonstrated superior decision-making and organizational skills
- Demonstrated commitment to professional development
- Proficiency in Windows, Microsoft Word 95, Power Point
- Must possess own car, a current Ontario Driver’s License and a clean driving record
- Demonstrates excellence in communication both verbal and written
- Analysis and problem assessment
- Demonstrated pattern of good attendance and work record.
- Demonstrates an understanding of and commitment to SHNs comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area.
- Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
