Site: Toronto Western Hospital
Department: General Internal Medicine (GIM)
Scale/Grade: E0-06
Salary Range: $33.56 – $48.05 Per Hour (start rate determined in accordance with the collective agreement requirements)
Hours: 37.5 Hours per Week, Monday to Friday, 9:00am to 5:00pm
Status: Temporary Full-Time (Approx. length of contract 1 year)

As a part of the GIM team the Registered Nurse will work with the care teams to identify complex patients at high risk for multiple admissions, and facilitate a smooth transition between their inpatient care group, the community and their Primary Care Physicians. The Registered Nurse will improve the coordination of care between acute care and the community through the following actions and attributes:

  • Coordinating the development of Care Plans in collaboration with the GIM IP teams, Community care and Primary Care Physicians
  • Familiarity with concepts of chronic disease management and patient self-management. Understanding of primary care context and delivery
  • Ability to deal with issues identified on a post-discharge phone call according to an agreed upon algorithm
  • Facilitation and coordination of direct access to Medical Imaging and other investigative tests, and specialized subspecialty clinics.
  • Facilitation of direct admissions to specialized clinics with physician guidance
  • Working collaboratively with the patient and family, primary care and Community care teams to assist with preparing care plans uniquely designed for each patient to enable him/her to successfully manage their care at home
  • Collaboration with the project team to support the development of communications systems and processes that support patient transitions after hospital visits and hospitalizations
  • Collaborating with project team to develop systems to ensure information (i.e. discharge summaries, test results) is shared between acute care, community care and primary care

QUALIFICATIONS:

  • Current Registration as a Registered Nurse with the College of Nurses of Ontario
  • Basic Cardiac Life Support (B.C.L.S.) certification
  • 2 years recent – Acute Care Medical/Surgical Nursing experience
  • Experience working with older patients with complex medical needs
  • Expert health assessment skills
  • Strong Medical Nursing background preferred
  • Previous Medical Nursing experience preferred
  • Must be flexible and able to adapt to changing environments and travel outside of hospital; must be able to meet the physical requirements of the position
  • Computer literacy
  • Strong organizational skills
  • Excellent interpersonal and decision making skills
  • B. Sc. N. preferred
  • Registration with the RNAO preferred
