Job Description:
The Social Worker will actively participate in the delivery of care and ongoing program development for the Paediatric Diabetes Education Centre. The Paediatric Diabetes Education Centre is comprised of a multi-disciplinary team that assists children/adolescents (aged 0 – 18) and their families as they adjust to and cope with a diabetes diagnosis. The Social Work position involves working with an inter-professional team, conducting psychosocial assessments, and providing mental health support to help children and their families lead active and healthy lives.
Responsibilities include:
- Individual and family psychosocial assessments
- Provide child and caregiver/family counseling
- Case formulation and treatment planning with inter-professional team members
- Collaboration/communication with the inter-professional health care team is essential
- Consult and collaborate with community partners for treatment planning
- Actively involved in developing and executing program initiatives
- May include the supervision of trainees and learners
- Integrates clinical and research responsibilities as the team is required to demonstrate an evidence-based approach to this clinical activity and to create, utilize and/or disseminate new knowledge
- Collect and evaluate metrics
Qualifications
- MSW or BSW, registered with the Ontario College of Social Workers and Social Service workers required
- 2 – 3 years’ experience with paediatric patient care populations
- Basic diabetes knowledge
- An understanding of the psychosocial impact of living with a chronic illness
- experience with individual and family psychosocial assessments
- group facilitation experience preferred
- excellent assessment, counseling, problem-solving, and community-resourcing skills
- demonstrated ability to work well both autonomously and within a multi-disciplinary team
- strong interpersonal, and communication skills
- strong leadership skills
- excellent project management and program development abilities
- demonstrated ability to work effectively under pressure
- demonstrated facilitation and consensus-building skills
- must be able to meet the travel requirements of the position
